ALL TEAMS MUST UPLOAD DOCUMENTS TO GOTSOCCER BY WEDNESDAY AUGUST 5 2020

COMPETITIVE CHECK-IN INFORMATION

• One (1) Copy of official team roster
  • US Club or North Texas

• Medical Release Form 
  •  (One per player including guest players)
    (Medical Releases do not need to be Notarized)
  • Please have these with you at each game - do not need to be up-loaded to gotsoccer

• Guest Player Release
  • North Texas Teams - Guest Player Release 
  • Non-North Texas Teams, paperwork per your State Association rules

• ID Cards (Player Passes) 
  • Non-North Texas Teams and US Club teams please have with you at each game

• Travel Roster or Travel Permit
  • Non-North Texas Teams Only

• GotSoccer Roster (Mandatory): Deadline – 8/7/18
  • Every team is required to up-date their GotSoccer Roster, do not forget to add your guest players
  • If a player is not on the GotSoccer Roster, he/she will not be allowed to play. 

 

Game Day Roster/Reports
The Tournament will provide game reports – winning team turns in signed game report to the headquarters tent. In case of a tie – the home team is responsible to turn in the report.

 

ACADEMY/JUNIORS TEAM CHECK-IN INFORMATION

• North Texas Academy Team Tournament Roster 
  • Signed by a North Texas Registrar

• OR a US Club official roster 
  • Must have player cards

• Player Academy Registration Form (if using North Texas Academy Tournament Roster)
  • A copy of every player’s form on your roster.

• Medical Release Form
  • Medical Release for every player on roster.
    (Medical Releases do not need to be Notarized)

• GotSoccer Roster(Mandatory)
  • Every team is required to update their GotSoccer Roster.
  • If a player is not on the GotSoccer roster, he/she will not be allowed to play.
